October 2004 — Forklift Operator Crushed

OSHA is investigating yet another fatal forklift accident. This time the accident resulted in the death of a 40-year old employee in Florida who had worked for the company for six years. Investigators believe the operator of the forklift struck a pothole in the pavement near standing water that caused the forklift to overturn onto him.

Investigators were told by an employee that he heard a co-worker driving the forklift at what sounded like a high rate of speed before the crash. The man ran outside and saw the victim pinned underneath a support bar on the forklift. The man was pronounced dead at the scene.

Lessons learned: High speed and poor road conditions equal trouble, whether you are driving a truck, auto or forklift. Vehicle safety needs to be an ongoing topic for training and communication programs.
